This essay analyzes two types of patient‐experience data to broaden and deepen understanding of trust in health care. Analysis of patients’ open‐ended comments shows a close connection between patients’ feelings of trust and their intent to recommend providers and provider organizations—a global measure to evaluate patients’ perceptions of care experiences. Patients’ comments also reveal the bidirectional building of trust between the patient and the caregiver. Trust gets built when patients perceive their caregivers to trust their knowledge of their bodies as (...) well as when caregivers demonstrate caring behaviors that earn the patients’ trust. Patients’ ratings of a closed‐ended survey item on “confidence in provider” create the greatest differentiation for the global measure of patient experience—whether patients did or did not recommend a practice or provider. The essay also discusses related findings on pre‐visit friction and the use of humor by the caregiver to expand understanding of trust. (shrink)

Our book Relevance (Sperber and Wilson 1986) treats utterance interpretation as a two-phase process: a modular decoding phase is seen as providing input to a central inferential phase in which a linguistically encoded logical form is contextually enriched and used to construct a hypothesis about the speaker's informative intention. Relevance was mainly concerned with the inferential phase of comprehension: we had to answer Fodor's challenge that while decoding processes are quite well understood, inferential processes are not only not understood, but (...) perhaps not even understandable (see Fodor 1983). Here we will look more closely at the decoding phase and consider what types of information may be linguistically encoded, and how the borderline between decoding and inference can be drawn. It might be that all linguistically encoded information is cut to a single pattern: all truth conditions, say, or all instructions for use. However, there is a robust intuition that two basic types of meaning can be found. This intuition surfaces in a variety of distinctions: between describing and indicating, stating and showing, saying and conventionally implicating, or between truth-conditional and non-truth-conditional, conceptual and procedural, or representational and computational meaning. In the literature, justifications for these distinctions have been developed in both strictly linguistic and more broadly cognitive terms. The linguistic justification goes as follows (see for example Recanati 1987). Utterances express propositions; propositions have truth conditions; but the meaning of an utterance is not exhausted by its truth conditions, i.e. the truth conditions of the proposition expressed. This article looks at the feminist activism of particular women in the ancestry of the eminent Canadian sociologist, Dorothy E. Smith, and at the archival data that confirm the traces of their influence found in her theory-building. Using the method of interpretative historical sociology and a conceptual framework drawn from Marx called the `productive forces', the article examines the feminist theology of her Quaker ancestor, Margaret Fell, and the militant suffrage activism of her mother and her grandmother, Dorothy Foster Place (...) and Lucy Ellison Abraham, respectively. The article argues that the household labour of the remarkable women in her family line became a `productive force' that facilitated her imagining of the feminist theory, `the standpoint of women'. Background Since the UK Abortion Act (1967), women have travelled from Ireland to the UK for legal abortion. In 2011 >4000 women did so. Knowledge and attitudes of medical students towards abortion have been published, however, this is the first such report from Ireland. Objective To investigate medical students’ attitudes towards abortion in Ireland. Methods All medical students at the University of Limerick, and physicians who graduated from the university within the previous 12 months, were invited via email to complete (...) an anonymous online survey. The questionnaire comprised 17 questions. Quantitative and qualitative analyses were performed. Results Response rate was 45% (n=169; 55% women; 88.2% <30 years of age; 66.7% Irish; 29.2% North American). Outcomes were: abortion should not be legally available (7.1%), abortion should be allowed in limited circumstances only (35.5%), abortion should be legally available upon request (55%). 72.8% of respondents were moderately/strongly prochoice (74% of women/71% of men/72% and 76% of Irish and North American respondents, respectively). Students aged >30 years were less likely to be prochoice (55%). While 95.2% believed that education on abortion should be offered within medical school curricula, 28.8% stated that they would decline to terminate pregnancies even if legally permitted. While 58.8% indicated that they might perform legal abortions once qualified, 25.7% would do so under limited circumstances only. Conclusions The majority of participants wanted education regarding abortion. Despite being predominantly prochoice, considerably fewer students, irrespective of nationality, indicated that they would perform abortions. (shrink)

Shaping Public Theology, which is also edited by Paeth along with E. Harold Breitenberg Jr. and Hak Joon Lee, is a helpfully representative compilation of Stack-house’s own writing across the span of his long and prolific career. As such, it demonstrates both the generous aspirations and the significant limitations of “public theology” as Stackhouse envisioned it, as a theologically informed discourse that should be equally accessible to Christians, people of other faiths, and people of no faith or religious background. These essays locate him firmly within the strand of Christian thought inhabited by Ernst Troeltsch, Walter Rauschenbusch, and Paul Tillich, and they also reflect his significant and ongoing disagreements with some of his contemporaries, including Alasdair MacIntyre and Stanley Hauerwas. The second section, “Developing a Method for Public Theology,” provides a useful summary of Stackhouse’s distinctive theological method, defining public theology and its relationship to individuals, communities, institutions, and states. The third and final section, “Constructing a Global Public Theology,” turns to one of the major focal points of Stackhouse’s work, examining the role of public theology in the context of globalization. The text concludes with a brief summary contributed by the editors, a response from Stackhouse describing both how his thought has changed over the years and the gaps that he perceives in his work, and a comprehensive bibliography.As a collection, Shaping Public Theology provides the reader with a broad overview of the predominant themes in Stackhouse’s work. The essays demonstrate his abiding concern for an apologetic and universal framework for Christian ethics as well as his emphasis on the covenantal basis for that framework. Stackhouse’s affinity for both the Reformed tradition and the ideals of the Social Gospel movement are also evident throughout the text. In addition many of the essays reflect his ongoing engagement with the theme of “the global” and with globalization not only as a locus of critique but also as the vehicle for “the prospect of a world-comprehending culture, with a new recognition of certain common moral standards” (162).Whether these ideas are theologically helpful, however, is a matter of long-standing disagreement that will not be resolved in the limited space of this review. By treating the covenant between the Triune God and Israel as merely one instance of a “universal moral law,” and by moving from that particular to the universal through the doctrine of the Trinity, Stackhouse participates in a significant—and significantly problematic—strain of thought within the Christian imaginary (67). While he may reject the particularity espoused by George Lindbeck, Hauerwas, et al., he remains grounded within his own particularity with its own decidedly fraught history: that of the modern, progressive, Western civilizing project. As a work of moral theology, this text does have significant limitations. Deirdre Wilson provides a reflective overview of a volume devoted to the historic application of relevance-theoretic ideas to literary studies. She maintains a view argued elsewhere that the putative non-propositional nature of literary effects are an illusion, a view which dates to Sperber and Wilson : “If you look at [non-propositional] affective effects through the microscope of relevance theory, you see a wide array of minute cognitive [i.e., propositional] effects.” This paper suggests an alternative, that modern-day humans have two (...) apparently different modes of expressing and interpreting information: one of these is a system in which propositional, cognitive effects dominate; the other involves direct, non-propositional effects. The paper concludes by describing two ways such affects might be assimilated into relevance theory. The first, to accept that humans are much more than merely cognitive organisms; the second, to rethink quite radically what we mean by cognition. (shrink)

For a century and a half, the artists and intellectuals of Europe have scorned the bourgeoisie. And for a millennium and a half, the philosophers and theologians of Europe have scorned the marketplace. The bourgeois life, capitalism, Mencken’s “booboisie” and David Brooks’s “bobos”—all have been, and still are, framed as being responsible for everything from financial to moral poverty, world wars, and spiritual desuetude. Countering these centuries of assumptions and unexamined thinking is Deirdre McCloskey’s _The Bourgeois Virtues_, a magnum (...) opus that offers a radical view: capitalism is good for us. It is how China and India began to embrace neoliberal ideas of economics and attributed a sense of dignity and liberty to the bourgeoisie they had denied for so long. The result was an explosion in economic growth and proof that economic change depends less on foreign trade, investment, or material causes, and a whole lot more on ideas and what people believe. A partir da década de 1980, o impulso de Wolin de diagnosticar as falhas da democracia americana ao conectar o liberalismo ao totalitarismo se manifesta de uma nova maneira. Ele observa que, no processo de lutar ostensiva- mente contra o totalitarismo durante a Guerra Fria, o próprio governo americano passou a incorporar uma forma totalizante de poder. Essa crítica culmina em seu diagnóstico de uma crise do século (...) XXI: a condição neoliberal de “totalitarismo invertido”. Aqui Wolin argumenta não apenas que o sistema político americano compartilha algumas características com o totalitarismo, mas também que está começando a constituir uma nova versão do totalitarismo emergindo de dentro, e não em reação ao liberalismo. São dois aspectos de uma mesma questão: em primeiro lugar, a universalidade no sentido arquitetônico, correspondente à totalidade; em segundo lugar, a determinação absoluta do indivíduo singular. G. E. Moore was a central figure in twentieth-century philosophy. Along with Russell and Wittgenstein, he pioneered analytic philosophy, and his Principia Ethica shaped the contours of twentieth-century ethics. Indeed, until the publication of Rawls's A Theory of Justice, no single book in moral philosophy was to equal Principia's influence. Unfortunately, however, Principia Ethica has so dominated critical discussions of Moore's work that even experts on his moral philosophy have tended to ignore his Ethics, which he published eight years later. (...) But Ethics is Moore's only other book on moral philosophy, and one of only a handful of post-Principia publications dealing with ethics. Its detailed discussions of utilitarianism, free will, and the objectivity of moral judgements find no real counterpart in Principia while its account of right and wrong and of the nature of intrinsic value deepen our understanding of his moral philosophy. The republication of Ethics thus rounds out our understanding of Moore's ethical thought. But the book's value goes beyond its historical or scholarly interest. A short but philosophically rich text, Ethics stands independent of Principia and repays careful study in its own right. By raising a number of fundamental questions in ethics, questions that remain live today, by proffering clear, credible, and often innovative answers to them, and by doing so with a philosophical skill that is still impressive, Moore's short book is a minor classic. Almost a century after its original publication, it still amply rewards those who read it.This new edition of Moore's Ethics includes his essay 'The Nature of Moral Philosophy' as well as editorial notes, an introduction, and a guide to further reading. Because corporate social responsibility (CSR) is potentially beneficial for companies, it is important to understand the factors that improve a company’s CSR practice. Scholars hypothesize that facilitating learning organization characteristics, which are divided in characteristics at the organizational and the operational level, may improve CSR implementation. These characteristics stimulate companies and their members to be critical, learn from the past, and embrace change, but there is limited empirical evidence of this approach. This study addresses this gap by surveying 280 CSR (...) professionals and performing bootstrap mediation analyses to test multiple hypotheses. Learning organization characteristics at the organizational level, play a key role in supporting CSR implementation: leadership for learning, system connection, and group learning show a direct relationship with CSR implementation. It is striking that the role of the learning organization characteristics at the operational level is only indirect; the organizational characteristics mediate their relationship with CSR implementation. Little is known about how employees on husbandry farms perceive animal welfare and the factors influencing the relationship between them and the animals they engage with in their daily work. Reporting the findings of qualitative interviews with 23 employees on five Danish farms (mink, dairy and pig production), this paper describes how the employees viewed animal welfare, and discusses how they dealt with animal welfare issues in their daily work. Four distinct rationales for animal welfare were identified. 1) Animal welfare (...) was supported by concerns about production and health, and could be negotiated – especially when it came to the ability of the animals to perform natural behaviour. 2) Animal welfare was connected with the working conditions on the farm. 3) The employees’ views about animal welfare were affected by working conditions over which they had no influence. 4) An awareness of the condition of the animals was seen as obviously needed in relation to production, but a deeper attachment to some animals was also seen. A specific challenge is presented by the increasingly diverse workforce in farming, with one third of the employees on Danish farms coming from abroad. If farm owners are not able to integrate these employees, there is a risk of creating a second-tier of foreign workers who are isolated. Furthermore, it was seen that negative working conditions can be taken out on the animals, or that animal welfare can come to be seen as unimportant as compared with human welfare. (shrink)
